Eye Care for Animals is available to assist you and your veterinarian with treatment of your pet’s eyes. While you may contact us by phone, we encourage your veterinarian to use our referral form. This will provide a record of your pet’s medical history and any previous treatments. You may then fax, mail, or hand deliver it to the Eye Care for Animals practice of your next appointment.
